INTRODUCTION AND AIMS: The outcomes of endoscopic submucosal dissection (ESD) in the esophagus have not been assessed in our country. Our primary aim was to analyze the effectiveness and safety of the technique. MATERIAL AND METHODS: Analysis of the prospectively maintained national registry of ESD. We included all superficial esophageal lesions removed by ESD in 17 hospitals (20 endoscopists) between January 2016 and December 2021. Subepithelial lesions were excluded. The primary outcome was curative resection. We conducted a survival analysis and used logistic regression analysis to assess predictors of non-curative resection. RESULTS: A total of 102 ESD were performed on 96 patients. The technical success rate was 100% and the percentage of en-bloc resection was 98%. The percentage of R0 and curative resection was 77.5% (n=79; 95%CI: 68%-84%) and 63.7% (n=65; 95%CI: 54%-72%), respectively. The most frequent histology was Barrett-related neoplasia (n=55 [53.9%]). The main reason for non-curative resection was deep submucosal invasion (n=25). The centers with a lower volume of ESD obtained worse results in terms of curative resection. The rate of perforation, delayed bleeding and post-procedural stenosis were 5%, 5% and 15.7%, respectively. No patient died or required surgery due to an adverse effect. After a median follow-up of 14months, 20patients (20.8%) underwent surgery and/or chemoradiotherapy, and 9 patients died (mortality 9.4%). CONCLUSIONS: In Spain, esophageal ESD is curative in approximately two out of three patients, with an acceptable risk of adverse events.

Background: Self-expanding metal prostheses improve dysphagia in patients with incurable esophageal cancer (EC). New stents have been introduced, and chemoradiotherapy has been implemented for EC, changing patients' risk profiles. It is unknown whether this has affected palliation with stents. Patients and methods: Retrospective study in three centers in Medellín-Colombia; patients undergoing placement of palliative esophageal prostheses for malignant dysphagia (1997-2022). Major and minor complications after implantation, the influence of oncological therapies, and survival were evaluated for 1997-2009 (n = 289) and 2010-2022 (n = 318). Results: 607 patients underwent esophageal prostheses; 296 (48.8%) became complicated. It was higher in the second period (52.5% vs. 48.1%), as were major complications (20.8% vs. 14.2%, p = 0.033), with no differences in minor complications (33.9% vs 31.8%, p = 0.765). Also, 190 (31.3%) patients presented with recurrent dysphagia, stable in both periods. Migration increased over time (from 13.1% to 18.2%, p = 0.09). The most common minor adverse event was pain, increasing over time (from 24.9% to 33.95%, p < 0.01), and associated factors were chemoradiotherapy, absence of fistula, and squamous cell carcinoma. Acid reflux decreased in the second group (p = 0.038). Twelve percent of patients required another intervention for feeding. Survival was not impacted by time and use of stents. Conclusions: Stents are an alternative in non-surgical malignant dysphagia, although recurrent dysphagia has not decreased over time. Minor stent-related complications are increasing in association with the implementation of chemoradiotherapy.


Antecedentes: Las prótesis metálicas autoexpandibles mejoran la disfagia en pacientes con cáncer esofágico (CE) incurable. En las últimas décadas se han introducido nuevos tipos de stents y se ha implementado la quimiorradioterapia para el CE, generando cambios en los perfiles de riesgo de los pacientes. Se desconoce si estos cambios han afectado la paliación con stents. Pacientes y métodos: Estudio retrospectivo en tres centros de Medellín-Colombia; pacientes sometidos a colocación de prótesis esofágicas paliativas para disfagia maligna (1997-2022). Se evaluaron en dos períodos: 1997-2009 (n = 289) y 2010-2022 (n = 318), complicaciones mayores y menores después del implante, la influencia de las terapias oncológicas y la sobrevida. Resultados: Se evaluaron 607 pacientes sometidos a prótesis esofágicas. 296 (48,8%) se complicaron, y fue mayor en el segundo periodo (52,5% frente a 48,1%), al igual que las complicaciones mayores (20,8% frente a 14,2%, p = 0,033), sin diferencias en complicaciones menores (33,9% frente a 31,8%, p = 0,765). 190 (31,3%) pacientes presentaron disfagia recurrente, estable en ambos períodos. La migración aumentó con el tiempo (de 13,1% a 18,2%, p = 0,09). El evento adverso menor más frecuente fue dolor, que aumentó con el tiempo (de 24,9% a 33,95%, p < 0,01), y los factores asociados fueron quimiorradioterapia, ausencia de fístula y carcinoma de células escamosas. El reflujo ácido disminuyó en el segundo grupo (p = 0,038). El 12% de pacientes requirieron otra intervención para alimentarse. No se impactó la sobrevida con el tiempo y uso de stents. Conclusiones: Los stents son una alternativa en la disfagia maligna no quirúrgica, aunque la disfagia recurrente no ha disminuido con el tiempo. Las complicaciones menores relacionadas con el stent van en aumento, asociadas a la implementación de la quimiorradioterapia.

RESUMO

Sleeve gastrectomy has become the most performed bariatric surgery technique in the world. This bariatric technique has been related to the appearance of gastroesophageal reflux and recently with de novo Barrett's esophagus. It is not clear that this leads to an increased incidence of esophageal adenocarcinoma. In this review we analyze the current scientific literature to try to answer the true incidence of Barrett's esophagus and adenocarcinoma after sleeve gastrectomy, and whether these data should make us change the indications for this technique.

OBJECTIVE: Patients with head and neck squamous cell carcinoma (HNSCC) have an increased risk of second esophageal neoplasms. The aim of the present study is to evaluate the incidence, risk factors and prognosis of second esophageal neoplasms in patients with HNSCC. METHODS: A retrospective study of 4711 patients with index tumor in the oral cavity, oropharynx, hypopharynx or larynx between 1985 and 2020 was conducted. RESULTS: During the period analysed 149 patients (3.2%) had a second esophageal neoplasm. The incidence of second esophageal neoplasia was 0.42% per year and remained nearly constant throughout the follow-up period. According to the results of a multivariate study, the risk factors related to the appearance of second esophageal neoplasms were a history of high alcohol consumption and the location of the primary tumor in the oropharynx or hypopharynx. The 5-year disease-specific survival rate in patients with a second esophageal neoplasm, calculated from the diagnosis of this second neoplasm, was 10.5%. CONCLUSIONS: Patients with a HNSCC have an increased risk of developing of a second esophageal neoplasm. The risk factors associated with the appearance of a second esophageal neoplasm were severe alcohol consumption and the location of the primary tumor in the oropharynx or hypopharynx.

Introducción: El tratamiento principal del cáncer esofágico es la esofagectomía. Objetivo: Determinar la morbilidad y mortalidad de los pacientes operados de cáncer esofágico. Métodos: Se realizó un estudio observacional, descriptivo y transversal en 87 pacientes operados de cáncer esofágico en el Servicio de Cirugía General del Hospital Provincial Docente Saturnino Lora de Santiago de Cuba durante el período comprendido de 2014 a 2018. Resultados: Dos pacientes en estadio II (100,0 por ciento) recibieron tratamiento quirúrgico curativo y 12 en estadio III (17,9 por ciento). Recibieron tratamiento quirúrgico paliativo 55 (82,1 por ciento) enfermos en estadio III y 18 (100,0 por ciento) en estadio IV. Predominó la técnica transhiatal de Denk-Sloan-Orringer aplicada en 10 (71,4 por ciento) pacientes. La gastrostomía predominó como proceder paliativo para alimentación en 53 (76,2 por ciento) pacientes. Se reportaron 45 complicaciones; el 53,3 por ciento de tipo médica, en las que prevalecieron las respiratorias: bronconeumonía (13,3 por ciento) y distrés respiratorio (11,1 por ciento). En cambio, el 46,7 por ciento de las complicaciones fueron de tipo quirúrgicas: infección del sitio operatorio (20,0 por ciento), seguida de la fuga anastomótica (15,6 por ciento). Fallecieron 16 (18,4 por ciento) pacientes del total de la serie. Las causas de muerte predominantes fueron el distrés respiratorio (31,3 por ciento) y la disfunción múltiple de órganos (25,0 por ciento). Conclusiones: La esofagectomía abierta o mínimamente invasiva se erige como el tratamiento quirúrgico de elección para el tratamiento del cáncer esofágico con intención curativa, proceder con elevada morbilidad y mortalidad a escala mundial. Los resultados de esta investigación coinciden con los reportados en la literatura médica nacional y extranjera(AU)


Introduction: The main treatment for esophageal cancer is esophagectomy. Objective: To determine the morbidity and mortality of patients operated on for esophageal cancer. Methods: An observational, descriptive and cross-sectional study was carried out with 87 patients operated on for esophageal cancer in the general surgery service of Hospital Provincial Docente Saturnino Lora, of Santiago de Cuba, during the period from 2014 to 2018. Results: Curative surgical treatment was received by 2 patients (100.0 percent) in stage II and 12 patients (17.9 percent) in stage III. Palliative surgical treatment was received by 55 ill patients (82.1 percent) in stage III and 18 ill patients (100.0 percent) in stage IV. There was a predominance of the Denk-Sloan-Orringer transhiatal technique, applied in 10 (71.4 percent) patients. Gastrostomy predominated in 53 (76.2 percent) patients as a palliative procedure for feeding. Forty-five complications were reported, 53.3 percent of which were medical, with respiratory complications prevailing: bronchopneumonia (13.3 percent) and respiratory distress (11.1 percent). On the other hand, 46.7 percent of the complications were surgical: surgical site infection (20.0 percent), followed by anastomotic leak (15.6 percent). Out of the total series, 16 (18.4 percent) patients died. The predominant causes of death were respiratory distress (31.3 percent) and multiple organ dysfunction (25.0 percent). Conclusions: Open or minimally invasive esophagectomy stands out as the surgical treatment of choice for esophageal cancer with curative purposes, being a procedure with high morbidity and mortality worldwide. The results of this research coincide with those reported in the national and foreign medical literature(AU)

OBJECTIVE: To assess the accuracy of the diagnostic tests for a correct clinical tumor staging in localized esophageal cancer (EC). METHOD: Retrospective observational study of patients who underwent esophagectomy for cancer in a referral hospital between January 2003 and September 2019. Those patients who received neoadjuvant treatment were excluded in order to avoid bias from downstaging effects. The preoperative stage was compared with the pathological stage of the surgical specimen. Computed tomography (CT) , endoscopic ultrasound (EUS) and positron emission tomography (PET) were evaluated. The pT stage was correlated with the tumor length described in the esophagram (EG). RESULTS: Among the 63 patients included, the clinical staging was correct in 16 (global accuracy 25.4%), it was overstaged in 21 (33.2%) and understaged in 26 (41.3%). For cT staging, the accuracy of EUS was higher than that of CT (46.6% and 34.9%, respectively), specially for early stages. EG tumor length correlated with pT stage (p < 0.05). For cN staging, PET had the highest sensitivity (50.0%) and negative predictive value (75.0%). CONCLUSIONS: Despite the multiple diagnostic tools used, the global accuracy of clinical staging in localized EC is still a challenge. The lack of a test that stands out significantly from the others reinforces the need to use them in a complementary way.


OBJETIVO: Evaluar la exactitud diagnóstica para el estadiaje clínico del cáncer de esófago (CE) localizado. MÉTODO: Estudio observacional retrospectivo de los pacientes esofagectomizados por CE en un hospital de referencia entre enero de 2003 y septiembre de 2019. Se excluyeron aquellos que recibieron neoadyuvancia para evitar sesgos de infraestadiaje. Se comparó el estadio preoperatorio con el estadio patológico de la pieza quirúrgica. Se evaluaron la tomografía computarizada (TC), la ecoendoscopia (EUS) y la tomografía por emisión de positrones (PET). El estadio pT se correlacionó con la longitud tumoral descrita en el esofagograma (EG). RESULTADOS: De los 63 pacientes incluidos, el estadiaje clínico fue correcto en 16 (exactitud 25.4%), con sobreestadiaje en 21 (33.2%) e infraestadiaje en 26 (41.3%). Para el estadiaje cT, la EUS fue superior a la TC (exactitud 46.6% y 34.9%, respectivamente), en especial para estadios precoces. La longitud tumoral del EG se correlacionó con el estadio pT (p < 0.05). Para el estadiaje cN, la PET tuvo la mayor sensibilidad (50.0%) y el mayor valor predictivo negativo (75.0%). CONCLUSIONES: A pesar de las múltiples herramientas diagnósticas empleadas, la exactitud diagnóstica en el CE localizado es limitada. La ausencia de una prueba que destaque de manera significativa refuerza la necesidad de emplearlas de forma complementaria.

Introducción: El cáncer de esófago se manifiesta clínicamente en etapas avanzadas y presenta gran letalidad. Objetivo: Caracterizar a los pacientes operados de cáncer de esófago según variables epidemiológicas, clínicas, diagnósticas e histopatológicas. Métodos: Se realizó un estudio observacional, descriptivo y transversal de 87 pacientes con cáncer de esófago, operados en el Servicio de Cirugía General del Hospital Provincial Docente Clinicoquirúrgico Saturnino Lora de Santiago de Cuba, en el periodo 2014-2018. Resultados: Dicha afección predominó en pacientes del sexo masculino (94,2 %) entre los 65 y 74 años de edad (37,9 %); el tipo histológico más frecuente fue el carcinoma epidermoide (89,6 %) y la localización en el tercio medio (49,4 %). Los hábitos tóxicos que primaron fueron el tabaquismo (87,4 %) y el consumo de alcohol (75,8 %). La disfagia y la pérdida de peso se manifestaron en 73,5 y 66,6 % de la serie, respectivamente. El diagnóstico se realizó mediante imágenes, tales como esofagograma, tomografía contrastada de tórax, abdomen y pelvis; así como por medio de endoscopia y biopsia. Conclusiones: Se demostró que los hombres son los más afectados y es más frecuente el diagnóstico en mayores de 60 años de edad. Se observó el vínculo con el hábito de fumar y el consumo de alcohol. La disfagia es el síntoma más común y tardío de la enfermedad. El estudio de las características clínico-histológicas de los pacientes con cáncer de esófago permite un adecuado enfoque diagnóstico de esta afección, así como el desarrollo de acciones de salud preventivas sobre los principales factores de riesgo identificados.


Introduction: The esophagus cancer is clinically manifested in advanced stages and presents great lethality. Objective: To characterize the patients operated on esophagus cancer according to epidemiological, clinical, diagnostic, histological and pathological variables. Methods: An observational, descriptive and cross-sectional study of 87 patients with esophagus cancer was carried out; they were operated in the General Surgery Service of Saturnino Lora Teaching Clinical Surgical Provincial Hospital in Santiago de Cuba, in the period 2014-2018. Results: This affection prevailed in patients from the male sex (94.2 %) between the 65 and 74 years (37.9 %); the most frequent histhologic type was the squamous cell carcinoma (89.6 %) and the localization in the mean third (49.4 %). The toxic habits that prevailed were nicotine addiction (87.4 %) and consumption of alcohol (75.8 %). The dysphagia and loss of weight showed in 73.5 and 66.6 % of the series, respectively. The diagnosis was carried out by means of images, such as esophagogram, chest, abdomen and pelvis contrast tomography, as well as by means of endoscopy and biopsy. Conclusions: It was demonstrated that men are the most affected and it is more frequent the diagnosis in those over 60 years. The link with the habit of smoking and consumption of alcohol was observed. The dysphagia is the most common and late symptom of the disease. The study of clinical, histological and pathological characteristics of patients with esophagus cancer allows an appropriate diagnostic approach of this affection, as well as the development of preventive health actions about the main identified risk factors.

Introducción: El cáncer de esófago y cardias es una enfermedad grave, en el que más de tres cuartos de los enfermos que se tratan están en fase avanzada. En Cuba existe experiencia con las prótesis esofágicas originales o construidas en el país. Objetivo: Evaluar los resultados de la experiencia de los hospitales universitarios "Comandante Manuel Fajardo" y "Miguel Enríquez" con las prótesis transtumorales en el cáncer de esófago no resecable. Métodos: Se realizó un estudio observacional, descriptivo, retrospectivo, tipo serie de casos, con una muestra de 197 enfermos con cáncer avanzado del esófago en los Hospitales Universitarios "Comandante Manuel Fajardo" y "Miguel Enríquez" tratados con prótesis por presentar un cáncer avanzado del esófago en el período 1995-2020. Resultados: Predominó el sexo masculino, las edades más afectadas fueron la sexta y séptima década. La localización más frecuente del tumor fue en tercio inferior del esófago y cardias y el tipo histológico el carcinoma epidermoide. En el 71 por ciento se utilizaron prótesis rígidas de tubos endotraqueales; en el 20,8 por ciento prótesis de Heering y en el 8,2 por ciento prótesis autoexpansible. En el Hospital Manuel Fajardo la prótesis que más se utilizó fue la rígida de construcción artesanal y en el Hospital Miguel Enríquez la de Heering. La menos utilizada fue la autoexpansible. Se presentó un 10,1 por ciento de complicaciones, sin mortalidad quirúrgica. Conclusiones: Aunque las prótesis autoexpansible son de fácil colocación por endoscopia, es un método ideal pero costoso. La prótesis plástica rígida industrial o artesanal también puede ofrecer calidad de vida sin disfagia, evitan que el enfermo fallezca con una ostomía abdominal(AU)


Introduction: Esophageal and cardia cancer is a serious disease, in which more than three quarters of the treated patients are in advanced stage. In Cuba there is experience with original esophageal prostheses or others domestically built. Objective: To evaluate the results of the experience of the Comandante Manuel Fajardo and Miguel Enriquez university hospitals with transtumoral prostheses in unresectable esophageal cancer. Methods: An observational, descriptive, retrospective, case-series type study was carried out with a sample of 197 patients treated with prosthesis for advanced esophageal cancer in the university hospitals Comandante Manuel Fajardo and Miguel Enríquez in the period 1995-2020. Results: The male sex predominated, while the most affected ages were the sixth and seventh decades. The most frequent tumor location was the lower third of the esophagus and cardia, while squamous cell carcinoma was the most frequent histological type. Rigid prostheses of endotracheal tubes were used in 71 percent; Heering prostheses, in 20.8 percent; and self-expanding prostheses, in 8.2 percent. The most commonly used prostheses at Manuel Fajardo and Miguel Enriquez hospitals were the rigid prosthesis of handmade construction and the Heering prosthesis, respectively. The least used was the self-expanding prosthesis. There was a 10.1 percent of complications, with no surgical mortality. Conclusions: Although self-expanding prostheses are easy to place by endoscopy, it is an expensive method, even being ideal. The industrial or handmade rigid plastic prosthesis can also offer quality of life without dysphagia, avoiding the patient's death with an abdominal ostomy(AU)

AIM: To assess the clinical utility of PERCIST criteria and changes in [18F]FDG PET/CT quantitative parameters as prognostic factors for progression-free survival and cancer-specific survival (CSS) in patients with esophageal cancer treated by chemoradiotherapy. MATERIAL AND METHODS: Fifty patients (48 men) diagnosed with esophageal cancer were retrospectively evaluated over a 7.5-year interval. PERCIST criteria were used to assess response to neoadjuvant therapy. Variations in the metabolic parameters maximum SUV (SUVmax), metabolic tumor volume (MTV) and total lesion glycolysis (TLG) between pre- and post-treatment PET/CT studies were also determined. ROC curves, Kaplan-Meier method and Cox regression model were used for the analysis of prognostic factors and survival curves. RESULTS: The average follow-up was 26.8 months, with 40 recurrences-progressions and 41 deaths. Survival analysis showed statistically significant differences in CSS curves for PERCIST criteria and variation of MTV and TLG. PERCIST criteria were the only independent predictor in the multivariate analysis. Neither SUVmax nor tumor size were predictors for any of the assessment criteria. CONCLUSION: Application of PERCIST criteria as well as change in MTV and TLG from PET/CT studies proved to be prognostic factors for CSS in patients in our setting treated for esophageal cancer. The results could help to personalize treatment.

Introducción: La esofagectomía es uno de los procedimientos con más alta morbilidad posoperatoria en cirugía oncológica digestiva. Objetivo: Describir las complicaciones relacionadas con la esofagectomía subtotal programada según variables de interés. Métodos: Se realizó un estudio descriptivo y transversal en los Servicios de Cirugía General de los hospitales "Saturnino Lora" y "Juan Bruno Zayas" de Santiago de Cuba, desde 2010 hasta 2019. La población estuvo conformada por 81 pacientes, de la cual se reclutó una muestra aleatoria de 68 enfermos sometidos a esofagectomía subtotal programada por cáncer de esófago. Fueron investigadas las siguientes variables: presencia de complicaciones posoperatorias y tipo de técnica quirúrgica utilizada. Además, se clasificaron en grados según Dindo Clavien. Resultados: Todos los enfermos fueron operados por técnica abierta mediante esofagectomía subtotal programada, donde la resección mediante la técnica de Ivor-Lewis se realizó en el 69,2 por ciento. Para todas las técnicas quirúrgicas empleadas predominó la atelectasia (39,7 por ciento) como complicación posquirúrgica, y entre las infecciosas la neumonía en 30 enfermos (44,1 por ciento), mientras que para las complicaciones quirúrgicas no infecciosas el neumotórax fue mayormente incidente, para el 25,0 por ciento. El mayor número de complicaciones se ubicó en el grado II de Dindo Clavien. Conclusiones: La presencia de complicaciones posquirúrgicas de la enfermedad es notable, en correspondencia a lo reflejado por la literatura médica. Existió relación significativa entre la presencia del hábito de fumar, de enfermedad pulmonar obstructiva crónica y la de toracotomía con algunas de las complicaciones encontradas(AU)


Introduction: Esophagectomy is one of the procedures with the highest postoperative morbidity in digestive cancer surgery. Objective: To describe the complications related to scheduled subtotal esophagectomy according to variables of interest. Methods: A descriptive and cross-sectional study was carried out in the General Surgery Services at Saturnino Lora and Juan Bruno Zayas hospitals in Santiago de Cuba, from 2010 to 2019. The population consisted of 81 patients, out of which a random sample of 68 patients undergoing elective subtotal esophagectomy for esophageal cancer was selected. The variables investigated were presence of postoperative complications and type of surgical technique used. In addition, they were classified in grades according to Clavien-Dindo. Results: All the patients were operated by open technique by programmed subtotal esophagectomy, the resection by the Ivor-Lewis technique was performed in 69.2 percent. For all the surgical techniques used, atelectasis (39.7 percent) prevailed as a postsurgical complication, and among the infectious complications, pneumonia prevailed in 30 patients (44.1 percent), while for non-infectious surgical complications, pneumothorax was mostly incidental for 25.0 percent. The highest number of complications was in Clavien-Dindo grade II. Conclusions: The presence of post-surgical complications of the disease is notable, corresponding to what is reflected in the medical literature. There was significant relationship between the presence of smoking, chronic obstructive pulmonary disease and thoracotomy with some of the complications found(AU)

Introducción: La morbilidad y la mortalidad continúan siendo altas a escala mundial como consecuencia de la esofagectomía programada por cáncer de esófago. Objetivo: Identificar los factores predictores de complicaciones posquirúrgicas y de muerte, la estadía hospitalaria y las causas de muerte en la población estudiada. Método: Se efectuó un estudio de cohorte de 81 pacientes con diagnóstico de cáncer de esófago, atendidos en los hospitales Saturnino Lora y Dr. Juan Bruno Zayas Alfonso de la provincia de Santiago de Cuba desde enero de 2010 hasta diciembre de 2019, de los cuales se escogió una muestra aleatoria de 68. Para identificar los factores predictivos asociados con las complicaciones y la mortalidad se definieron 2 cohortes de enfermos: los expuestos y los no expuestos a los factores de interés, tales como la realización de toracotomía, las reintervenciones, además de las complicaciones médicas y quirúrgicas infecciosas. Se utilizó el método estadístico de bondad de ajuste de Hosmer-Lemeshow con un nivel de significación α= 0,10. Resultados: Si se realiza toracotomía, la probabilidad de que se desarrollen complicaciones quirúrgicas infecciosas es 2,3 veces mayor que si no se lleva a cabo el proceder; asimismo, el riesgo de fallecer por dichas complicaciones (p=0,024), en contraposición a cuando no se presentan, asciende a 370,0 % (IC 90 %: 1,5-14,8). Conclusión: La realización de toracotomía se establece como factor predictor de complicaciones y la presencia de reintervenciones y de complicaciones médicas y quirúrgicas infecciosas como predictores de muerte en la esofagectomía programada.


Introduction: Morbidity and mortality continue being high worldwide as consequence of the esophagectomy programmed due to esophagus cancer. Objective: To identify the predictor factors of postsurgical and death complications, hospitalization and death causes in the studied population. Method: A cohort study of 81 patients with diagnosis of esophagus cancer was carried out. They were assisted in Saturnino Lora and Dr. Juan Bruno Zayas Alfonso hospitals in the province of Santiago de Cuba from January, 2010 to December, 2019, of which a random sample of 68 was chosen. To identify the prediction factors associated with the complications and mortality 2 cohorts of sick patients were defined: the exposed and those not exposed to interest factors, such as the thoracotomy realization, reinterventions, besides the infectious medical and surgical complications. The statistical method of Hosmer-Lemeshow was used with a significance level of α = 0,10. Results: If thoracotomy is carried out, the probability that infectious surgical complications are developed is 2.3 times more that if it is not carried out; also, the risk of dying due to this complications (p=0.024), in opposition to when they are not presented, ascends to 370.0 % (IC 90 %: 1.5-14.8). Conclusion: The thoracotomy realization is established as predictor factor of complications and the presence of reinterventions and infectious medical and surgical complications as death predictors in the programmed esophagectomy.

RESUMEN Introducción: El cáncer de esófago es la octava neoplasia diagnosticada con más frecuencia a nivel mundial, con 456 mil nuevos casos al año. Objetivo: Caracterizar a pacientes con cáncer de esófago que se les realiza esofagectomía subtotal programada. Métodos: Se realizó un estudio observacional, descriptivo de una serie de casos en los servicios de cirugía general de los Hospitales Provincial "Saturnino Lora" y General "Juan B. Zayas" de Santiago de Cuba desde 2010 al 2019. El universo estuvo conformado por 81 pacientes con diagnóstico de cáncer de esófago, de los cuales se seleccionaron 68 por cumplir con los criterios de inclusión. Fueron investigadas variables epidemiológicas, clínicas y quirúrgicas. Se calcularon porcentajes, media aritmética y desviación estándar. Resultados: Se observó predominio de los hombres (88,2 %); dentro de los factores epidemiológicos hubo predominio del hábito alcohólico (75,0 %) y del tabaquismo (58,8 %), la disfagia fue el síntoma más frecuente (48 enfermos). Las lesiones tumorales localizadas en el tercio medio e inferior fueron las más frecuentes. Predominó el carcinoma epidermoide en 52 pacientes y la localización del tumor en el tercio medio esofágico. En cuanto al tipo de resección quirúrgica la más utilizada fue la de McKeown (87,2 %). Conclusión: El cáncer de esófago es más frecuente en varones de la tercera década de la vida, fumadores, consumidores de alcohol y con mal estado nutricional. El patrón histológico predominante es el carcinoma epidermoide vegetante. La esofagectomía transhiatal es la variante técnica más utilizada en su tratamiento quirúrgico.


ABSTRACT Introduction: Esophageal cancer is the eighth most frequently diagnosed neoplasm worldwide with 456 thousand new cases per year. Objective: To characterize patients with esophageal cancer who undergo programmed subtotal esophagectomy. Methods: An observational, descriptive study of a case series was carried out in the general surgery services of the Provincial Hospital "Saturnino Lora" and General "Juan B. Zayas" of Santiago de Cuba from 2010 to 2019. The universe was made up of 81 patients diagnosed with esophageal cancer, of which 68 were selected agreeing inclusion criteria. Epidemiological, clinical and surgical variables were investigated. Percentages, arithmetic mean and standard deviation were calculated. Results: A predominance of men was observed (88.2 %); among the epidemiological factors, there was a predominance of alcoholic habit (75.0 %) and smoking (58.8 %), dysphagia in 48 patients was the most frequent symptom. Tumor lesions located in the middle and lower third were the most frequent. Squamous cell carcinoma predominated in 52 patients and the tumor was located in the middle third of the esophagus. Regarding the type of surgical resection, the most used was that of McKeown (87.2 %). Conclusion: Esophageal cancer is more frequent in men in the third decade of life, smokers, chronic alcoholics and poor nutritional status. The predominant histological pattern is vegetative squamous cell carcinoma. Transhiatal esophagectomy is the most widely used technical variant in its surgical treatment.

RESUMEN Introducción: El carcinoma epidermoide constituye la variedad más frecuente de cáncer esofágico. Se considera poco frecuente en la práctica clínica y su evolución es muy agresiva. Objetivo: Describir las manifestaciones clínicas y diagnóstico de un paciente con carcinoma epidermoide de esófago. Caso clínico: Paciente masculino de 46 años de edad, que comenzó con fiebre, tos y disfonía; apareció disfagia 20 días después. Se identificó un tumor por exámenes imagenológicos y se indicó biopsia con toma de muestra, mediante endoscopia superior, de manera que se confirmó la enfermedad. Conclusiones: El cáncer esofágico es una entidad infrecuente. Su síntoma cardinal, la disfagia, no se evidencia al inicio de la enfermedad y su diagnóstico se realiza a través de estudios imagenológicos e histológicos.


ABSTRACT Introduction: Squamous cell carcinoma constitutes the most frequent variety of esophageal cancer. It is considered rare in clinical practice and its evolution is very aggressive. Objective: To describe the clinical manifestations and diagnosis of a patient with squamous cell carcinoma of the esophagus. Clinical case: 46-year-old male patient, who began with fever, cough and dysphonia. Dysphagia developed 20 days later, the tumor was identified by imaging tests and a biopsy was indicated with sampling by upper endoscopy so that the disease was confirmed. Conclusions: Esophageal cancer is a rare entity. Its cardinal symptom, dysphagia, is not evident at the beginning of the disease and its diagnosis is made through imaging and histological studies.

RESUMEN Antecedentes: en la última década ha comenzado a investigarse el uso de la captación de fluorescencia mediante luz infrarroja para la visualización de ganglios linfáticos en tumores de estómago y esófago. Objetivo: evaluar la factibilidad de la evaluación del drenaje linfático de cáncer de esófago mediante el uso de fluorescencia y verde de indocianina (ICG). Material y métodos: se realizó un estudio prospectivo que incluyó pacientes con tumores de la unión gastroesofágica resecables (estadios I, II y III). Antes de comenzar la cirugía se inyectaron por endoscopia 4 mL de ICG doblemente diluida en agua estéril (1,25 mg/mL = 5 mg) en la submucosa del esófago en los cuatro cuadrantes (1 mL por cuadrante) alrededor del tumor. Resultados: se incluyeron en total 6 pacientes. En todos ellos se logró identificar el drenaje linfático del tumor hacia la primera estación ganglionar: en 6/6 (100%), el drenaje linfático con fluorescencia se detectó en las estaciones ganglionares N°s 3 y 7 (curvatura menor y arteria gástrica izquierda). En ningún paciente se identificó fluorescencia en ganglios mediastinales. Conclusión: la visualización del drenaje linfático de tumores de la unión gastroesofágica mediante el uso de fluorescencia con ICG es factible.


ABSTRACT Background: Over the past decade, fluorescence imaging with infrared light has been used to visualize lymph nodes in tumors of the stomach and esophagus. Objective: The aim of our study was to evaluate the feasibility of evaluating lymphatic drainage in esophageal cancer using fluorescence and indocyanine green (ICG). Material and methods: We conducted a prospective study of patients with resectable tumors of the gastroesophageal junction (stage I, II and III). Before surgery, 4 mL of ICG double diluted in sterile water (1.25 mg/mL = 5 mg) were injected via endoscopy into the esophageal submucosa in the four quadrants (1 mL per quadrant) around the tumor. Results: A total of 6 patients were included. Lymphatic drainage from the tumor to the first lymph node station was identified in all patients: in 6/6 (100%), fluorescent lymphatic drainage was detected in nodal stations number 3 and 7 (lesser curvature and left gastric artery) Fluorescence was not identified in the mediastinal lymph nodes in any patient. Conclusion: Visualization of lymphatic drainage of gastroesophageal junction tumors to the lesser curvature nodes using fluorescence imaging is feasible.

RESUMEN Introducción: El cáncer de esófago es una enfermedad descrita desde hace 2 000 años y en la actualidad ocupa el octavo lugar en incidencia a nivel mundial. Objetivo: Describir las características clínico-histológicas de los pacientes con cáncer esofágico del Hospital Nacional "Guido Valadares" de Timor Oriental. Método: Se realizó un estudio observacional-descriptivo y retrospectivo en 106 personas con diagnóstico endoscópico e histológico de cánceres esofágicos atendidos en la Unidad de Endoscopia del servicio de Gastroenterología de dicha institución, desde junio de 2016 hasta mayo de 2021. Se analizaron variables como: grupo etario, sexo y otros factores de riesgo (manifestaciones clínicas, tiempo de comienzo de los síntomas, localización, tipo endoscópico y tipo histológico). Resultados: El cáncer de esófago fue más frecuente en hombres de 60 y más años de edad (65,1 %). El tabaquismo y la ingestión de alcohol fueron los factores de riesgo predominantes con el 64,2 % y 57,5 %, respectivamente. La mayoría de los casos se diagnosticaron entre los 3 y 6 meses del inicio de los síntomas, la disfagia el principal motivo de consulta (98,1 %). Desde el punto de vista endoscópico predominó el tipo polipoide o vegetante localizados en el tercio medio esofágico (45,3 %), constituyendo el carcinoma de células escamosas el tipo histológico más frecuente. Conclusiones: El estudio de las características clínico-histológicas de los pacientes con cáncer esofágico permite un adecuado enfoque diagnóstico de esta enfermedad, así como el desarrollo de acciones de salud preventivas sobre los principales factores de riesgo identificados.


ABSTRACT Introduction: Esophageal cancer is a leading cause of cancer-related illness that has been described for two thousand years and currently set at the eighth place in incidence worldwide. Objective: To describe the clinical-histological features in patients with esophageal cancer at the Guido Valadares National Hospital in Timor-Leste. Method: An observational-descriptive and retrospective study was conducted in 106 patients with esophageal cancer related endoscopic and histological diagnosis. All of them attended in the Endoscopy Unit at the Gastroenterology service, from June 2016 to May 2021. Variables such as age group, sex and other risk factors like (clinical manifestations, the onset of symptom, location, endoscopic type and histological type) were analyzed. Results: Esophageal cancer was more frequent in male aged 60 and older (65.1%). Smoking and alcohol intake were the predominant risk factors (64.2% and 57.5%, respectively). Most of cases were diagnosed between 3 and 6 months after the onset of symptoms, with dysphagia as the main reason for consultation (98.1%). From the endoscopic point of view, polypoid mass or vegetating lesions found in the middle third of the esophagus predominated (45.3%), with squamous cell carcinoma as the most frequent histological type. Conclusions: Study of clinical-histological features in patients with esophageal cancer allows an adequate diagnostic approach to this illness, as well as the development of preventive health actions on the main risk factors identified.


RESUMO Introdução: O câncer de esôfago é uma doença descrita há 2.000 anos e atualmente ocupa o oitavo lugar em incidência mundial. Objetivo: Descrever as características clínico-histológicas dos pacientes com câncer de esôfago no Hospital Nacional "Guido Valadares" em Timor Leste. Método: Foi realizado um estudo observacional-descritivo e retrospectivo em 106 pessoas com diagnóstico endoscópico e histológico de câncer de esôfago atendidas na Unidade de Endoscopia do serviço de Gastroenterologia da referida instituição, no período de junho de 2016 a maio de 2021. Variáveis como: foram analisadas: faixa etária, sexo e outros fatores de risco (manifestações clínicas, tempo de início dos sintomas, localização, tipo endoscópico e tipo histológico). Resultados: O câncer de esôfago foi mais frequente em homens com 60 anos ou mais (65,1%). O tabagismo e o consumo de álcool foram os fatores de risco predominantes com 64,2% e 57,5%, respectivamente. A maioria dos casos foi diagnosticada entre 3 e 6 meses do início dos sintomas, sendo a disfagia o principal motivo de consulta (98,1%). Do ponto de vista endoscópico, predominou o tipo polipóide ou vegetativo localizado no terço médio do esôfago (45,3%), sendo o carcinoma espinocelular o tipo histológico mais frequente. Conclusões: O estudo das características clínico-histológicas de pacientes com câncer de esôfago permite uma adequada abordagem diagnóstica desta doença, bem como o desenvolvimento de ações preventivas de saúde sobre os principais fatores de risco identificados.

AIM: To determine the utility of [18F]FDG PET/CT quantitative parameters as prognostic factors for the response to neoadjuvant treatment, progression-free survival (PFS) and cancer-specific survival (CSS) in patients with esophageal squamous cell carcinoma (SCC). MATERIAL AND METHODS: Thirty patients (29 men) diagnosed with SCC were retrospectively evaluated over a 6-year interval. Metabolic parameters were determined: maximum SUV (SUVmax), mean SUV (SUVmed), metabolic tumor volume (MTV) and total lesion glycolysis (TLG) from baseline PET/CT study. After treatment with chemotherapy and/or radiotherapy, response to treatment and patient survival were assessed. The comparison of parameters between groups of responders and non-responders was carried out using a Mann-Whitney U test ROC curves and the Kaplan-Meier method were used for analysis of prognostic factors and survival curves. RESULTS: The average follow-up was 22.4 months, with 22 recurrence-progressions and 25 deaths. Significant differences were demonstrated between responders and non-responders with respect to tumor size, MTV and TLG. Survival analysis found significant differences for SCE and CSS depending on these three parameters. CONCLUSION: Metabolic parameters MTV and TLG, and tumor size were prognostic factors for neoadjuvant treatment response, PFS, and CSS in patients diagnosed with SCC. Neither SUVmax nor SUVmed were predictive for any of the evaluation criteria. Results could help to personalize patient treatment.

RESUMEN Introducción: El cáncer de esófago es una enfermedad presente a escala mundial, sobre todo en naciones en vías de desarrollo. Se realizó una revisión bibliográfica en bases de datos biomédicas mediante el motor de búsqueda de información Google académico. Se seleccionaron 24 de 62 artículos; 22 (91,6 %) del último quinquenio, publicados en español e inglés, relacionados con el objetivo propuesto. Objetivo: Describir algunos aspectos del cáncer de esófago, con énfasis en los epidemiológicos, etiopatogénicos y del tratamiento quirúrgico. Desarrollo: El cáncer de esófago es más frecuente en varones, con proporción hombre/mujer de 6:1, aunque en poblaciones de alto riesgo la incidencia es igual en ambos sexos; aunque algunos refieren que la relación es de 3 a 10 hombres por cada mujer, en dependencia de la zona geográfica. Las áreas de alta frecuencia son: Asia, África Central y del Sur. El carcinoma epidermoide y el adenocarcinoma representan el 95,0 % de todos los tumores esofágicos. La esofagectomía abierta o mínimamente invasiva constituye el pilar básico del tratamiento quirúrgico. Conclusiones: En la etiología del cáncer de esófago, los factores moleculares, genéticos y ambientales juegan un papel importante. El diagnóstico en etapas avanzadas predomina debido a lo tardío de la aparición de los síntomas y a las insuficiencias en el cribaje. El tratamiento quirúrgico constituye un pilar fundamental para los tres primeros estadios de la enfermedad, acompañada o no de quimio-radioterapia adyuvante o de neoadyuvancia (en el estadio II).


ABSTRACT Introduction: Esophageal cancer is a disease present worldwide, especially in developing nations. A bibliographic review was carried out in biomedical databases using the Google Scholar search engine. 24 of 62 articles were selected; 22 (91.6 %) of the last five years, published in Spanish and English, related to the proposed objective. Objective: To describe some aspects of esophageal cancer, with emphasis on the epidemiological, etiopathogenic and surgical treatment. Development: Esophageal cancer is more common in men, with a male/female ratio of 6:1, although in high-risk populations the incidence is the same in both sexes; although some report that the ratio is between 3 and 10 men for every woman, depending on the geographical area. High frequency areas are: Asia, Central and South of Africa. Squamous cell carcinoma and adenocarcinoma represent 95.0 % of all esophageal tumors. Open or minimally invasive esophagectomy is the basic pillar of surgical treatment. Conclusions: In the etiology of esophageal cancer, molecular, genetic and environmental factors play an important role. Diagnosis in advanced stages predominates due to the late onset of symptoms and insufficient screening. Surgical treatment is a fundamental pillar for the first three stages of the disease, accompanied or not by adjuvant or neoadjuvant chemo-radiotherapy (in stage II).

Introducción: El cáncer en Cuba constituye un grave problema de salud debido a sus altas tasas de incidencia y mortalidad. Representa la primera causa de años de vida potencialmente perdidos y en el esófago es poco frecuente, su comportamiento es más agresivo y la supervivencia es baja. Objetivo: Caracterizar los pacientes operados por cáncer de esófago en Cuba en el periodo de 1988 al 2020. Métodos: Se realizó una investigación observacional clínico epidemiológica multicéntrica, no randomizada, en el periodo de 1988 al 2020. El universo y la muestra quedaron constituido por 595 enfermos de 9 centros hospitalarios. Algunas variables estudiadas incluyeron sexo, edad, variedad histológica, intervención quirúrgica realizada y presencia de comorbilidades asociadas. Resultados: Predominó el sexo masculino (66,1 por ciento), la edad promedio fue 62,8 años. La hipoproteinemia estuvo presente en el 46,1 por ciento. El 43,2 por ciento fueron fumadores y el 35,8 por ciento ingería bebidas alcohólicas. El epidermoide fue la variedad histológica más frecuente (64,6 por ciento). Las técnicas quirúrgicas más utilizadas fueron la de Ivor Lewis (68,4 por ciento) y la transhiatal (63,6 por ciento). Predominó la anastomosis en un plano, término-lateral, de localización torácica para el epidermoide (68,4 por ciento) y la cervical para el adenocarcinoma (71,4 por ciento), realizada de forma manual en cara posterior. Introduction: Cancer in Cuba is a serious health concern due to its high incidence and mortality rates. It represents the first cause of years of life potentially lost and. Esophageal cancer is rare, but its behavior is more aggressive and its survival rate is low. Objective: To characterize patients operated on for esophageal cancer in Cuba in the period from 1988 to 2020. Methods: A nonrandomized, multicenter, clinical-epidemiological and observational research was carried out in the period from 1988 to 2020. The universe and the sample consisted of 595 patients from nine hospital institutions. Some of the variables studied included sex, age, histological variety, surgical intervention performed, and presence of associated comorbidities. Results: There was a predominance of the male sex (66.1 percent). The average age was 62.8 years. Hypoproteinemia was present in 46.1 percent. 43.2 percent of patients were smokers and 35.8 percent drank alcoholic beverages. Epidermoid was the most frequent histological variety (64.6 percent). The most widely used surgical techniques were Ivor Lewis (68.4 percent) and transhiatal (63.6 percent) esophagectomies. There was a predominance of one-plane, termino-lateral anastomosis of thoracic location for epidermoid cases (68.4 percent) and the cervical one for adenocarcinoma cases (71.4 percent), performed manually on the posterior face. Conclusion: Esophageal cancer prevailed in men over 55 years of age, smokers and with hypoproteinemia. The most used surgical techniques were Ivor Lewis and transhiatal esophagectomies(AU)

Introducción: La fuga anastomótica es la principal complicación de la cirugía del esófago, con altos índices de incidencia y mortalidad. Objetivo: Identificar los factores involucrados en la aparición de la fuga anastomótica en pacientes operados de cáncer de esófago en Cuba en el periodo de 1988 al 2020. Métodos: Se realizó una investigación observacional clínico epidemiológica multicéntrica, no randomizada, en el periodo de 1988 al 2020. El universo y la muestra quedaron constituidos por 2844 y 595 enfermos de 9 centros hospitalarios. Algunas variables estudiadas incluyeron sexo, edad, variedad histológica, intervención quirúrgica realizada, comorbilidades y su interrelación con la fuga anastomótica. Resultados: La fuga anastomótica predominó en el grupo de mayores de 55 años (17,8 por ciento), en el sexo femenino 65 (10,9 por ciento), en los no fumadores 79 (13,3 por ciento) y en los que no ingerían bebidas alcohólicas con 100 pacientes (16,8 por ciento). En enfermos con niveles de albúmina normales 98 (16,5 por ciento), con proteínas bajas 94 (15,7por ciento) y cifras normales de hemoglobina 105 (17,6 por ciento). En la variedad histológica adenocarcinoma 65 (10,9 por ciento), la técnica quirúrgica transhiatal 69 (11,6 por ciento), realizada de forma manual 123 (20,7 por ciento), de localización cervical 111 (18,6 por ciento), en cara anterior 57 (9,6 por ciento), en un plano 78 (13,1 por ciento), en la variante término-lateral 120 (20,2 por ciento). Conclusiones: La fuga anastomótica predominó en mujeres mayores de 50 años, con adenocarcinoma, operadas con técnicas transhiatales, manuales, en un plano, termino-laterales, en cara anterior y con hipoproteinemia(AU)


Introduction: Anastomotic leak is the main complication of esophageal surgery, with high incidence and mortality rates. Objective: To identify the factors involved in the appearance of anastomotic leak in patients operated on for esophageal cancer in Cuba in the period from 1988 to 2020. Methods: A nonrandomized, multicenter, clinical-epidemiological and observational investigation was carried out in the period from 1988 to 2020. The universe and the sample consisted of 2844 and 595 patients from nine hospital centers. Some of the variables studied included sex, age, histological variety, surgical intervention performed, comorbidities, and their interrelation with anastomotic leak. Results: Anastomotic leak prevailed in the group aged over 55 years (17.8 percent), in the female sex (65; 10.9 percent)), in nonsmokers (79; 13.3 percent)) and in those who did not ingest alcoholic beverages (100, 16.8; as well as in patients with normal albumin levels (98; 16.5), with low proteins (94; 15.7 percent) and with normal levels of hemoglobin (105; 17.6 percent). In the histological variety of adenocarcinoma (65; 10.9 percent), there was a predominance of the transhiatal surgical technique (69; 11.6 percent), performed manually (123; 20.7 percent), of cervical location (111; 18.6 percent), in the anterior face (57; 9.6 percent), in one plane (78; 13.1 percent), and in the termino-lateral variant (120; 20.2 percent). Conclusions: Anastomotic leak prevailed in women aged over 50 years, with adenocarcinoma, operated on through transhiatal techniques, manually, in one plane, using the termino-lateral variant, in the anterior face and with hypoproteinemia(AU)

RESUMO

Cancer of the esophagus is an aggressive cancer with high mortality. Because of the esophagus's lack of serosa and its peculiar lymphatic drainage, esophageal cancer is diagnosed in advanced stages. The eighth edition of the TNM (2017) aims to standardize care for esophageal cancer throughout the world; it includes not only patients treated with esophagectomy alone, but also those receiving neoadjuvant chemotherapy and/or radiotherapy. One new development in the eighth edition is that it establishes separate classifications for different time periods, with pathologic stage groups for prior to treatment (cTNM), after esophagectomy (pTNM), and after neoadjuvant therapy (ypTNM). The combined use of endoscopic ultrasound, CT, PET-CT, and MRI provides the greatest accuracy in determining the clinical stage, and these techniques are essential for planning treatment and for evaluating the response to neoadjuvant treatment. Esophagectomy continues to be the main treatment; it is also the elective gastrointestinal surgery that has the highest mortality, and it carries the risk of multiple complications, including anastomotic leaks, pulmonary complications, technical complications, and functional complications.
